First visit procedure初めて受診される方

The Saitama University Hospital is an advanced treatment hospital. Before visiting our hospital for the first time, we recommend that you visit your previous doctor and obtain a referral including any treatment information. This is to help make your visit at Saitama Medical Hospital run more smoothly.

For patients visiting the hospital for the first time without a referral from another medical institution, a special fee of 5,500 yen for your first consultation will be charged. This fee will not be covered by your national health insurance plan.

当院は特定機能病院です。初めて受診される場合は、できるだけお近くの医療機関を受診され、診療情報を含む紹介状をご持参の上受診して下さい。当院での診療がより円滑に進みます。

紹介状をお持ちでない場合でも受診いただけますが、医療費の他に初診時選定療養費として、5,500円を自費でお支払いいただく事になりますのでご了承ください。

Upon your arrival, please complete the Consultation Application Form and submit it at the first-visit reception counter. Please submit your health insurance card and any referrals from any other medical institutions at this counter as well.

『外来診療申込書』に必要事項を記入し、保険証・紹介状を添えて、初診受付窓口へ提出してください。

Please note that the medical treatment fee is not covered if you do not submit your health insurance card.

Return visit再来の方

With an appointment予約がある方: Those with a Patient Registration Card (ID card) can simply check in by using the return visit reception machine located at each outpatient department counter. A numbered waiting ticket will be automatically issued from this machine.; 各科外来の受付機で受付できます。
自動再来受付機に診察券（IDカード）を入れ、予約受付票をお受け取りください。

Without an appointment予約がない方: Please come to the outpatient department counter between 8:30 am and 11:00 am and submit your Patient Registration Card (ID card). A waiting ticket will be issued by a staff member at this counter.; 午前8時30分～午前11時までに、各科外来窓口に診察券（IDカード）を提出し、受付票をお受け取りください。

Visiting only for an examination検査のみで来院の方: Submit your Patient Registration Card (ID card) and waiting ticket directly to an examination staff member.; 各検査部門の受付に直接診察券・予約票をお出しください。

Those seeking consultation after a long absence since their last visit AND with a first consultation from a different medical department from the one where they originally registeredしばらくぶりの受診・科初診（その科に初めてかかる）方

If you obtain any referrals from other medical institutions, please complete the administrative procedures for first-visit patients.

If you do not obtain any referrals from other medical institutions, please complete the administrative procedures at the reception counter of the department which you want to visit.

他院からの紹介状をお持ちの場合は、初診窓口での手続きとなります。
他院からの紹介状をお持ちでない方は、受診希望診療科の窓口での手続きとなります。

If you need assistance受診相談

If you do not know which department you should visit, or you are not sure which department your symptoms apply to, please ask for assistance at the general information desk on the first floor in the main building. The staff at the reception desk is always available to provide you assistance.

If you have lost your Patient Registration Card (ID card)診察券（IDカード）をなくされた方: We will reissue you a new Patient Registration Card (ID card) at the first-visit reception counter (Note: there will be a service charge for the new card).; 本館1階初診窓口で再発行致します。（有料）

If you forget to bring your Patient Registration Card (ID card)診察券を忘れた方: Please come to the first-visit reception counter and we will issue you a Patient Registration Card (paper version) which is valid only on that day.; 本館1階初診窓口で当日のみ有効の診察券（紙）を発行いたします。

About your consultation:診察について

Please be seated until your reception number appears on the electrical display in the waiting area of the department you are visiting. When your reception number appears on the electrical display, please enter the inside waiting room and report to the proper medical clinical room. If you are unsure where to go, an attending nurse will guide you to the correct consultation room.

各科案内表示盤にて診察のお呼びだしをします。
ご自身がお持ちの受付票の番号が表示盤に表示されたら、中待合室（診察室）へお入りください

Calculation and payment of your medical bill会計について

After the consultation, please wait for your patient file from department you are visiting. Afterward, please put your Patient Registration Card (ID card), your health insurance card, and parking ticket into your patient file.
Submit your patient file at payment counter on first floor in the south building, and you will receive a reception number ticket for payment.

Please be seated until your reception number appears on the electrical display, and pay your bill using one of the automatic cashier machines.

診察終了後、各科より会計ファイルをお渡ししますので、保険証・診察券・駐車券（お持ちの方）を一緒に入れ、南館1階会計ファイル提出窓口にお出しください。

会計ファイル提出窓口にて会計受付票を発行します。会計表示盤に番号が表示されましたら、外来自動精算機にて医療費をお支払い下さい。

Appointment for your next visit次回の予約について

If you make an appointment for your next visit at the end of your consultation, you will receive a sheet indicating your visit schedule along with a detailed statement of your medical bill. Please bring the appointment sheet with you during your next visit.

医療費のお支払い後発行される領収書に次回の予約票がついています。
次回来院時に切り離してお持ちください。
